Polk County High School Athletics

• Participants must have a physical each school year. Validity period starts June 1st. A student may not participate in any activity without a physical and the necessary paperwork turned into the specific coach.

• Participants must sign the Release and Consent forms before they can practice or play with a team

• The Polk County School Board provides secondary insuracne coverage to all athletes. Your carrier serves as the primary insurance.

• Please contact a coach in regards to questions about dates, times, scheduling, and off-season programs.

• Students may participate in summer programs only at the school they will be attending.
